Description:

Saint Francis Hospital, a member of Trinity Health Mid-Atlantic, is seeking an experienced Health Coach to join our ACO team!

As a Health Coach you will make frequent visits to your assigned group of practices in order to assist their patients with closing gaps in care and providing value-based care. It is a role that demands a solid foundational understanding of the healthcare delivery system for primary care practices, clinical plus administrative experience, and exceptional organizational skills. A successful Health Coach is passionate about population health, and highly mission oriented around providing better care to patients while reducing costs. At the Delaware Care Collaboration ACO, a Health Coach can impact the health of their community by transforming one practice at a time. It is a job for the healthcare professional who wants to make a difference and provide exceptional quality of care to patients.

The primary responsibilities:

  • Leading Population Health Management Quality Improvement (QI) processes in the practices and the processes encompassing population health management.

  • Communicates and coordinates with the healthcare teams in the development of tools for optimal patient outcomes, including assisting with the set-up of chronic condition management protocols in the electronic health record (EHR) and patient registry, patient database, Delaware Care Collaboration ACO Quality, and EHR report findings.

  • Supports continuous quality improvement, moving practices from reactive patient care to proactive patient care.

  • Works collaboratively with SFH referral and community resources to provide positive clinical outcomes for patients.

  • Works collaboratively with Providers, care teams and Colleagues to promote and obtain overall success of the medical home model within the practice.

  • Oversees the efforts to address the insurance gap reports (e.g., contractual performance metrics) and reporting.

Minimum Qualifications:

Education :

  • High School Diploma required; Bachelor's degree preferred

Licensure:

  • Licensed Medical Assistant preferred

  • Minimum of three years’ experience in a healthcare setting, preferably an ambulatory setting.

  • Previous experience in Healthcare Quality Metrics highly preferred

Skills

  • Strong Microsoft office suite software knowledge required

  • Knowledge of performance improvement, billing, coding, and documentation requirements highly desired

  • Knowledge of evidence-based behavioral change methodologies and motivational coaching techniques

  • Knowledge of patient safety principles

  • Excellent interpersonal, communication and writing skills are essential with the ability to relate and motivate effectively with a diverse group of patients and clinical staff

  • Understanding of Value Base Care, ACO, and HEDIS
