Home Care Clinical Manager Position Purpose:

The Clinical Manager manages the clinical and operational elements of the homecare program, achieving identified targets, metrics of: clinical outcomes/ratings at target or better, productivity, visits/episode, overtime, employee engagement/retention , budget targets.

What You Will Do:

  • Ensures that the appropriate Human Resource programs support and sustain agency growth.Programs include recruiting, employing and retaining qualified individuals to maintain staffing levels, staff development, safety, compliance, and employee relations.

  • Provides leadership and administrative support to agency operations.

  • Responsible for the management and supervision of staff providing direct client care, and assigned office colleagues

  • Oversees the delivery of quality client care through the application of health care knowledge in conjunction with physician orders and established policies, procedures and standards of practice.

  • Directs and supports orientation of managers, supervisors, and all direct client care staff, contract and contingent staff to the organization and to the delivery of clinicalservices.

  • Directs and supports in-service education and other continuing education activities.

  • Completes performance evaluations on all direct reports per policy.

  • Assures agency staff compliance with federal/state and accreditation regulations through record review, case conferencing, and mentoring.

  • Participate in leadership functions of the agency as a part of the management team.

  • Assures that accurate, complete client care documentation is completed timely, in preparation for billing.

  • Sets goals and priorities consistent with organizational mission and strategic plan.

  • Planning reflects appropriate input and interdepartmental or interdisciplinary consultation.

  • Understands, articulates, educates about and supports Trinity Health at Home (THAH) policies and procedures.

  • Plans, controls, and implements agency specific client care programs.

Minimum Qualifications:

  • Current license from the state in which practicing.

  • Bachelor’s Degree in nursing or therapy fieldrequired, Master's degree encouraged, (or if therapy, degree required from discipline)

  • Minimum three years' experience in a skilled homecare agency.

  • Must possess progressive supervisory or leadership experience.

  • Must have well developed interpersonal and communication skills to provide effective interface with clients, professional clinical staff and all other related team members.

  • Must possess a valid Driver’s License, from state in which practicing and have reliable transportation to and from the work site.
