RN Case Manager will be responsible for all aspects of case management for an assigned group of inpatients to determine the appropriateness of the continued stay, assist in the development of the plan of care; ensure that the plan is implemented in a timely manner, and identify the expected length of stay.

Qualifications:

Required:

• Must be a graduate from an accredited school of nursing

• Successful completion of all Orientation Program

Certifications and Licenses Required:

• Active Registered Nurse License in the state of New Jersey

• Basic Life Support (BLS) from the American Heart Association

• Case Management Certificate preferred

Scheduling Requirements:

• Per diem

• Day shift

• One (1) weekend in a month (Sat. & Sun.) Will accommodate if extra hours are requested for the weekends.

Essential Functions:

Understands financial responsibilities of the case management department and safeguards system resources by actively participating in utilization management and insurance communication. Collaborates closely with physician advisor. Participate in interdisciplinary rounds and facilitate communication with the interdisciplinary care team. Complete psychosocial assessments and implement patient-centered care plans that meet needs identified in assessments. Discharge planning activities: seamless and timely coordination of care including: referrals to home care, DME, SNF/ rehab placements, hospice, community resources, and transportation arrangements. Collaborates with Social Worker in all discharge planning. Participates in risk management referrals. Ensures that staff operate within compliance with all applicable policies, laws and regulations. Collaborate with department leadership to streamline processes and implement strategies to create more efficient delivery of care. Ensures compliance with regulatory documentation standards.
