• Primary functions may include: Clinical Interventions/Discharge, Care Planning Management, Fiscal Management, and Payer/Referral Management

• Responsible for the coordination of health care decisions by using a systematic approach to ensure treatment plans that improve quality and outcomes.

• Responsible for the promotion of cost-effective care within allotted time frame

• Assure payments of hospital-based services meet patient-related utilization of management criteria.

• Implementation of safe and appropriate discharge plans.

• Assiss the social work needs of the patient and provide intervention as part of the discharge planning process.

• Maintain accurate required clinical records.

• Demonstrate the skill in communicating concepts and information accurately orally or in writing.

• Maintain current knowledge of and abides by federal and state laws.

• Attends meetings and professional activities as required.

• Adheres to establish HIPPA confidentiality standards or patient/resident and client location information.

• Performs all other duties as assigned.

JOB REQUIREMENTS FOR THESocial Services Specialist:

• Master's Degree in Social Work from an accredited college or univiersity

• Valid State Social Workers Licence

• Post Social Work Master's Degree experience, working in relevent clinical settings

• CPR Certification
