Fedcap Licensed Master Social Worker in NY, United States

e the youth and young adult's lives. The Social Worker will alternate between Manhattan and Bronx sites as coordinated by and/or with their supervisor.

Essential Job Functions:

Provide one-to-one counseling as needed as well as group counseling sessions

Maintain regular contact with participants to meet program standards and achieve program retention

Ensure that referrals are made to appropriate internal and external services as required to meet the needs of the participants and their families

Write progress notes on sessions with program participants

Meet with program participants in group and one-on-one

Conduct thorough assessment, develop a psychosocial that informs the Individual Service Strategy (ISS) and assist with the development of educational and career-related goals collaboratively with participants based on the motivational interviewing process

Maintain open communication with program participants and program graduates through face-to-face contacts, phone contact, and social media such as Facebook

Communicate regularly with partners who refer and/or supervise participants.

Provide outcomes reporting

Maintain participant files, including progress notes and report on progress toward goals

Provide follow-up services for program graduates

Provide weekly reports to the program director

Participate in regularly scheduled supervision and ongoing training

Attend meetings and participate in professional development training offered

Education and Experience:

Licensed Master's Degree in Social Work (LMSW or LCSW (Licensed Clinical Social Worker).

Two (2) years of experience providing social work/mental health services to youth and young adults, including one year of experience with disconnecting youth and young adults within the last five years.

Experience working within a youth development model is a plus

Ability to evaluate and intervene in crisis situations

Bilingual in Spanish is a plus.
