Serenity Health & Wellness Corporation Licensed Independent Social Worker in Middleburg Heights, Ohio

As a Licensed Clinical Counselor, you will ensure that the emotional and clinical needs of Serenity Health & Wellness Corp. patients are consistently met and maintained by completing psychosocial assessments on all patients and determining individual holistic care plans of behavioral health. In collaboration with Physicians, and Care Coordinators. Our team of health care professionals designs treatment programs to address emotional and psychological health, as well as making sure all physical health needs are being met.

 

Your specific duties will include:

 

Conducting and documenting daily group therapy sessions    

Completing psychosocial assessments; participating in treatment planning and team meeting

Facilitating family and Individual therapy

Implementing and reviewing treatment

Discharge Planning

Interdisciplinary Team Meetings

Documentation within 24 hours

Light Case Management

 

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee must frequently lift and/or move up to 25 pounds and occasionally lift and/or move up to 50 pounds. While performing the duties of this Job, the employee is regularly required to speak and hear. The employee is frequently required to stand for prolonged periods; walk; use hands to finger, handle, feel and/or reach with hands and flex and extend arms. The employee may also be required to sit for extended periods. Up to 20% overnight travel may be required.
