PRIMARY FUNCTION:

To complete the eligibility, service authorization and provider support activities that are necessary to support individuals through the Medicaid Waiver and Supported Living programs.  Administering and authorizing services in a manner that is person centered and assures the rights, health and safety of the individuals, while in accordance with statues and regulations.

QUALIFICATIONS: Completion of a Bachelor's degree.  Must have at least 1 year paid work experience in human services or business. One year experience in budgeting, managing agency resources, overseeing contracts, or financial recordkeeping is preferred. Meets or exceeds necessary certification requirements. May be required to take additional training within 12 months of employment.

Must possess and maintain a valid Ohio Driver's License and be insurable.* * Must be able to transport self and/or individuals.

CERTIFICATION: DODD

DODD Certification - Service and Support Administrator required. May be required to obtain course work at own expense for initial certification as well as renewals.

Successful completion of all trainings necessary to carry out job including, but not limited to, Level of Care, Acuity Assessment Instrument and Ohio Developmental Disability Profile.
