POSITION SUMMARY:  

The Home-based Coordinator will assist in program development, marketing, staff training and service monitoring for the clinic’s home-based program. This position will take the lead role in coordinating the activities of the home-based team as well as collecting and report all necessary data to the supervisor. This position will also maintain a caseload with children in our home-based services. 

 

EDUCATION AND / OR EXPERIENCE:

  • Master’s degree in social work, psychology or other related human service field required. 

  • Registration/licensure as a social worker, counselor, or psychologist in the State of Michigan required.     

  • At least two years direct clinical experience in providing home-based services to families and youth required. This includes assessment, intervention, and treatment planning. 

     

    DUTIES AND RESPONSIBILITIES:

  • Work as part of an interdisciplinary team to provide assessment, treatment and support to youth and families in homebased programming. 

  • Provide anticipatory guidance to other professionals working with the homebased population. This includes training, technical assistance, and monitoring and coordination of assessments and interventions for children.

  • Provide comprehensive assessments, treatment planning and therapy services with measurable objectives in the home setting. Provide support and crisis intervention with families involved with the program. 

  • Assist in identification and reduction of cross-system barriers. 

  • Train others to assess the need for insight-oriented therapy versus behavior modification techniques for children.   

  • Identify and work closely with supervisor and staff on program development and client needs. 

  • Develop outcome measures for children in the homebased program. 

  • Work cooperatively as a member of a team with program staff, supervisors, and community referral sources. 

  • Participate in marketing and outreach activities, develop, and maintain referral relationships schools, medical agencies, and other referral sources. 

  • Maintain client files.  Submit timely and accurate documentation of services, billing data and required paperwork. 

  • Monitor program referrals and assignment of caseloads to the homebased program.  Responsible for working with team members, supervisor, child, families, and community contacts in a manner that is conducive to the philosophy and mission of the program and agency. 

  • Participate in individual and group supervision. 

  • Participate in on-going personal and professional development including in-service training, peer review, external workshops, and seminars. 

  • Accept and emulate the mission of SFS. 

  • Assist in finding and directing clients in case of an emergency such as a fire or tornado. 

  • Cooperate in any investigation related to personnel, licensing, accreditation, or other circumstances. 

  • Attend all agency and departmental meetings and training as required. 

  • Adhere to all Lifespan policies and procedures as it relates to documentation, productivity, training requirements, CMH guidelines, and confidentiality. 

  • Perform other related duties as assigned by supervisor or other management. 

     

    COMPETENCIES AND ABILITIES:

  • Knowledge of childhood social, emotional, and cognitive disabilities required along with basic knowledge of behavioral interventions to treat these disabilities. 

  • Willingness to provide group training/education to other professionals required. 

  • Ability to manage multiple priorities, function independently off site and demonstrate good follow through and problem-solving skills. 

  • Ability to demonstrate commitment, caring and respect for children and adults from diverse backgrounds who have multiple needs or problems. 

  • Ability to work cooperatively and responsibly as a member of a team with colleagues, supervisors, agency staff and community referral contacts. 

  • Ability to develop treatment plans and coordinate needed services to fulfill plans. 

  • Ability to role model appropriate child-handling techniques to parents and staff. 

  • Development and implementation of Individualized Plan of Service utilizing Person Centered Planning. 

  • Ability to make critical decisions independent of immediate supervision. 

  • Knowledge of community resources and linkages. 

  • Knowledge of theory and intervention with youth and family systems treatment and good understanding of child development, parent/child attachment and common medical and behavioral issues required.  

  • Competence in the delivery of crisis intervention services and in brief and time-limited therapy. 

  • Must be computer literate- fax, email, and Microsoft office at minimum. 

  • Ability to effectively, appropriately, and accurately communicate both orally and in writing. 

  • Ability to meet productivity expectations as assigned by program manager. 

  • Ability and willingness to abide by all Starfish Family Services confidentiality policies. 

  • Ability and desire for personal and professional growth, and skill development. 

  • Ability to demonstrate compassion and sensitivity and to respect the privacy and special needs of our clients.

  • Ability to represent the agency in a professional manner and develop new relationships with other community agencies.

     

    LICENSING AND OTHER REQUIREMENTS:  

  • Must have reliable mode of transportation Must possess a current valid State of Michigan chauffeur’s driver’s license and automobile insurance that is insurable. Must meet SFS and our insurance carrier guidelines for the transportation of children/ clients.
