Trinity Health Pulmonary Embolism Response Team Coordinator (PERT), Interventional Radiology in Fresno, California

Description:

Reporting to the Director Telemetry & Cardiovascular Services, Nursing Service Admin, this position as Pulmonary Embolism Response Team (PERT) is responsible to provide health care and coordination to Venous Thromboembolism (VTE) patients and caregivers, while in the hospital and during the first 90 days post discharge. As a high-risk group, VTE patients require complex coordinated care for overall reduction of subsequent illness that can lead to readmission to include exacerbated comorbid conditions.

This position is essential in a contemporary quality-based model for Cardiovascular services specific to VTE care. The Clinical Director for Cardiovascular Services will provide oversight of the functions and duties of this role.

  1. Bachelor of Science (BSN) in Nursing is required.

  2. Three (3) years of specialty experience in the designated clinical area is required.

  3. Current licensure as a Registered Nurse (RN) with the State of California is required.

  4. Current American Heart Association (AHA) Healthcare Provider CPR card is required.

  5. Current American Heart Association (AHA) Healthcare Provider Advanced Cardiac Life Support (ACLS) is required.

  6. Must demonstrate sound knowledge in cardiovascular health and related diagnostic services.

  7. Must have effective communication skills as well as strong clinical assessment, planning, intervention and evaluation skills.

Pay Range $51.12 - $74.12
