Summary:

Road Home Program: The National Center of Excellence for Veterans and Their Families (RHP) is dedicated to assisting and supporting current service members (active duty, Reserve and National Guard), veterans, and their families, with effective clinic services focused on treating the mental health diagnoses and difficulties caused by military service. RHP provides these services in collaboration with existing veteran and service member organizations.

The clinician will assess and treat veterans and their family members in a multidisciplinary outpatient practice setting which includes an intensive outpatient program for PTSD, individual, group, couples and family therapy. The clinician will provide therapy services to help stabilize the family and their service member or veteran. Insight and direction for RHP clients will be offered to ensure that clients are connected to other services and support in the community when needed. The clinician will work in collaboration with psychiatrists, psychologists, social workers, counselors and other members of Road Home Program (RHP)’s multidisciplinary team. Care will be focused on specific assigned patient populations including family, children, marriage/couples, and veterans. This position will exemplify the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Job Responsibilities:

  • Works directly with patients and their families to address unmet needs and provide the necessary social work services them achieve their

  • care goals.

  • Carries out comprehensive social work assessments by phone or in-person to develop a plan of care for the patient and/or family.

  • Connects patients and their families to relevant resources, both at Rush and in the community, to address their needs.

  • Provides care management and/or support programming for aforementioned population as outlined by Manager.

  • Documents all client interactions in either the Epic, LCM software and/or other spaces as indicated by Manager.

  • Maintains relationships with referring Rush clinics through consistent and timely communication.

  • Promotes Rush University System for Health clinical services by referring clients to relevant Rush clinics.

  • Represents the department at meetings, conferences or other public engagements on an as-needed basis.

Required Job Qualifications:

  • Master’s Degree in Social Work.

  • Licensed Social Worker (LCSW) in State of Illinois.

  • Two years’ experience in healthcare, care management, mental health and/or social services.

Preferred Job Qualifications:

  • Experience working with one or more of the following patient groups: family, children, couples, or veterans.

  • Previous work with service members, veterans and their families is strongly preferred.

  • Experience with psychotherapy evidence-based protocols.

  • Professional qualities such as initiative, self-reflection, commitment, responsiveness and adaptability.

  • Knowledge of care management, community-based interventions, assessment, counseling and/or motivational interviewing.

  • Ability and to apply inductive and deductive reasoning to information received and identifying relationships between seemingly unrelated events/information.

  • Computer literate (Microsoft Office and others).
