University of Virginia Clinical Social Worker - Stem Cell Transplant in Charlottesville, Virginia

Clinical Social Worker is responsible for conducting face to face sessions with patients in order to complete thorough assessments develop treatment plans and provide supportive counseling crisis intervention and brief therapy. Provides clinical services (caseload can consist of inpatients as well as family members on an individual couples and family basis). Responsible for serving populations with a variety of clinical needs particularly psychological stress related to health impairments chronic illness trauma death and dying marriage and family dysfunction as well as substance abuse. Works with victims of intimate partner violence abuse or exploitation and facilitate the identification and implementation of healthy boundaries and effective communication techniques. Works with patients and team members in an effort to maximize patients potential for the healthiest coping responses to their challenging situation including timely and safe discharge plans. Based on overall assessment and summary of patients and family needs makes appropriate community referrals for follow up as needed. Serves as a liaison between the medical staff and the patient/family and provide interventions designed to minimize patient stress and maximize coping skills to expedite recovery. Collaborates with all team members to maximize discharge planning and patient progression. Supervises bachelors and masters level students and acts as a liaison to the community through public speaking. Participates in clinical training and supervision as needed.

The BSW social worker in conjunction and collaboration with MSW/LCSW social workers screens and briefly assesses patients to identify their medical psychosocial and financial needs and appropriate services. Participates in ongoing team huddles with fellow Department of Social Work members to ensure quality of care in case assignment and level of intervention. Refers patients/families with identified social work needs to the MSW or LCSW social worker for comprehensive assessment. Works effectively within an interdisciplinary team model. Provides generalist social work services to patients and their families while maximizing utilization of personal family community and health care resources as related to management of the medical center process and disease continuum. Completes the Uniformed Assessment Instrument (UAI) and other government forms (local state and federal) as appropriate to access available benefits and services for patients. Arranges appropriate services makes referrals to community resources; educates patients and families regarding these resources. With the support of administrative assistant staff and collaboration with colleagues and leadership maintains a directory of current information regarding community resources eligibility requirements.

  • Completes initial screening patients in coverage area per department standards.

  • Coordinates with MSW/LCSW and Nurse Case Manager for high risk and complex patients to ensure continuity of care.

  • Explores patient/families current and projected financial resources for healthcare and essential needs (employment SSDI SSI Worker’s Compensation STD LTD and FMLA).

  • Acts as a patient advocate.

  • In addition to the above job responsibilities, other duties may be assigned.

Position Compensation Range: $24.24 - $38.78 Hourly

MINIMUM REQUIREMENTS

Education: Bachelor's degree in Social Work from a CSWE accredited social work program required.

PHYSICAL DEMANDS

Job requires sitting for prolonged periods, standing/traveling or use of assistive and climbing (stairs, steps). Proficient communicative, auditory and visual skills; Attention to detail, hear, speak, see, distinguish colors, read, ability to write legibly; Ability to lift/push/pull < 20lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
