Job Summary

Provides direct nursing care and support to tenants in accordance with the tenants’ service plan, and supervises the day-to-day nursing activities performed by tenant assistants. Provides medication management, chronic and acute disease monitoring, basic treatments and performs glucose monitoring. Keeps a safe and clean environment. Responsibilities also include monitoring and reporting tenant conditions, assisting with housekeeping, laundry and food service needs to provide holistic care for tenants.

Essential Duties and Responsibilities

• Uses nursing skills to monitor tenants’ baseline, recognize changes and implement interventions in accordance with the service plan.

• Administers drugs and other therapeutic and diagnostic measure to the tenant in accordance with physician orders, observes and chart the effects of these measures. Records appropriately on MAR and/or TAR.

• Has knowledge of and monitors both chronic and acute disease processes, reports to Director and physician to prevent and/or manage symptoms and exacerbations.

• Provides daily nursing care and other services according to nursing standards of care and documents appropriately. Includes but is not limited to bathing, dressing, grooming, transfers, ambulation, toileting, laundry, housekeeping and serving of meals.

• Communicates with Director and nurse supervisor frequently in regards to tenant needs, service plan changes, health status and procedures.

• Provides assistance to and from activities, appointments in accordance with service agreement while encouraging as much independence as appropriate for each tenant. Partners with pastoral and wellness teams to assure participation in resident selected activities.

• Performs accurate reporting and documentation using established procedures and training. Recognizes, records and reports unusual findings. Information includes but is not limited to blood sugars, vital signs, weight, behavior and mobility.

• Assists with resident admission, transfer or discharge following established procedures. Completes administrative duties required or as requested such as copying and faxing.

• Documents resident incidents, accidents, emergencies or unusual behavior.

• Consults with and keeps staff nurse informed of change in tenants’ condition as soon as possible.

• Anticipates and responds to tenants’ needs promptly. Partners with family and team to meet service plan needs.

• Performs other duties as necessary or assigned.

• Attends and participates in scheduled training and educational classes to maintain license as a practicing nurse. Attends all mandatory meetings and programs.

• Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.

• Complies with all applicable state and federal regulations.

• Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.

• Maintains the confidentiality of patient, resident, employee and organizational information.

Qualifications

Education and/or Experience

Certificate from college or technical school.

Language Skills

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to effectively present information in one-on-one and small group situations to customers, tenants, families and other employees or the organization.

Position Requirements, Certifications, License

Current unencumbered practical nursing license in the State of Iowa. Graduate from an approved practical nursing program.

Must have patience, tact, cheerful disposition and enthusiasm as well as the ability to handle tenant needs.

The employee must regularly lift and/or move up to 10 pounds, and may lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

While performing the duties of this job, the employee is occasionally exposed to wet and/or humid conditions, moving mechanical parts, toxic or caustic chemicals, and blood-borne pathogens. The employee may have exposure to combative residents. The noise level in the work environment is usually moderate.
