Summary

Social workers are critical and essential to the transplant care. This position is part of multiple dynamic medical and social work teams to assess, counsel, and provide education and psychosocial interventions to transplant recipients and their support networks. This position is primarily assigned to the Adult Liver Transplant Program, working with patients following transplant, but will provide coverage in other adult solid-organ groups to support a flexible workforce design. The majority of this work will occur in the outpatient setting however there are some inpatient responsibilities as well.

Responsibilities*

You will be responsible for:

  • Psychosocial assessment and intervention with post-transplant patients and families to address access to care and necessary medications, mental health and coping, substance use disorders and treatment needs, financial/insurance needs, medical-decision making and comprehension of medical recommendations, social support and caregiving needs.

  • Collaboration with interdisciplinary team for care coordination and programmatic initiatives.

  • On-call availability during clinic hours to provide immediate and/or urgent psychosocial intervention with Transplant Center patients and families.

  • Referral to and/or care coordination with appropriate resources for continuity of care and to address psychosocial needs.

  • Advocating for appropriate accommodations.

  • Participation in interdisciplinary meetings and rounds and presentation of psychosocial recommendations.

  • Participation in transplant continuous quality improvement initiatives.

  • Working with patients around issues of grief/loss, end-of-life decision making, treatment planning/goals of care, and adherence.

  • Development, evaluation, and collaboration on supportive programming to enhance the patient/family experience.

  • Cross coverage support within Transplant Social Work team.

  • Important skills for this position include: excellent verbal and written communication skills, precise and timely documentation, independent decision-making skills and the ability to prioritize multiple, competing tasks.

Required Qualifications*

  • A Master's degree in Social Work from a school accredited by the Council of Social Work Education.

  • A current license to practice social work in the State of Michigan. Full LMSW licensure is required for this position.

Desired Qualifications*

  • Previous Transplant Social Work experience.

  • Significant experience with substance use disorder populations and treatment; knowledge of SUD treatment resources.

  • Capable of making effective, independent judgment calls in a variety of demanding situations.

  • Significant experience with liver disease patients; medical social work and/or has transferrable skills.

This is a part time, 20 hour per week position.

Underfill Statement

This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification.

Background Screening

Background screenings are performed in compliance with the Fair Credit Report Act.
