On With Life RN MDS Coordinator in Ankeny, United States

This exempt position, available to Registered Nurses (RN), works Monday-Friday, with hours typically falling between 8a-4:30p, for an approximately total of 40 hours per week. This position is primarily based out of our post-acute inpatient program in Ankeny, but travel to our long-term program in Polk City, IA is required at least once a month to assist with those MDS’s as well.

There are no regular evening, weekend, or holiday requirements, but this position is part of our Nursing leadership on-call team so you may occasionally need to address issues or even potentially help cover the floor outside of your normal working hours.

We do annual raises based on budget capacity, and you also have the opportunity for a discretionary bonus at your anniversary. What does the RN MDS Coordinator do at On With Life?

Under the direction of the Director of Nursing and in partnership with other members of the transdisciplinary rehabilitation team, the RN MDS Coordinator:

  • Schedules and completes MDS, OBRA and OMRA assessments

  • Communicates MDS requirements and deadlines to other transdisciplinary team members

  • Finalizes comprehensive care plans

  • Assists with Nursing leadership on-call coverage

  • Provides direct RN care as needed

Qualified applicants must have:

  • Current licensure in good standing to practice as a Registered Nurse (RN) in the State of Iowa

  • Ability to maintain CPR certification (if you do not have this already, we will pay for it)
